Is your service accessible? (consider in general terms how you make your service accessible, in addition to any specific premises issues)

The club is run at Brookfields School. The school is accessible for children who are wheelchair users or who have mobility difficulties. The buildings have accessible show/changing/toilet facilities. For young people with a hearing impairment, some rooms are acoustically treated. The buildings and grounds are also accessible to young people with a visual impairment. Throughout the school there is manual handling equipment and staff working at the club have been trained in Manual Handling.

The young people who are eligible for having a place on the Holiday club must be a pupil at Brookfields School. However, we are not able to support pupils from the school at the Club who have complex medical needs. The Extended Services Manager and Club Leaders work together to determine if we can meet the needs of your child, taking further advice from teachers, Behaviour Support Staff, Sensory Resource staff, school nurses etc

How are referrals/applications made to your service? (include details of who can refer/apply and referral and application process)

Application forms are sent to families directly from the school. Parents may contact the Extended Services Manager at any time to find out more details about the clubs. Social Care may also request a place for a pupil from Brookfields School.

What is your service standard for responding to referrals/accessing services? (include timescales for responding to referrals and accessing services)

Applications need to be returned to the school by the date stated on the letter. We aim to get back to parents within 1 month to offer dates at the club
